Title of the Study: A Comparison of Teaching Strategies for First Aid and Farm Safety

Introduction/Purpose: The purpose of this research study is to see if students learn better if taught with a computer.

(Last Name) understand that I am being asked to participate in the study. I will be asked to complete a questionnaire, and possibly answer questions in a group, and/or perform a skill. Answers given in a group will be audio-recorded.

Procedures: This study will go on for about two months. First, I will write down answers to questions. Then I will be taught safety and first aid by a teacher or by the computer. About one week later, I will be asked to write down answers to questions again. If chosen I will sit in a group meeting and answer questions about how I liked learning from a teacher or computer. This meeting will be recorded on a tape-recorder. I may also be picked to perform first aid in a pretend situation.
Risks/Benefits: I will not be better off if I participate in the study, but I may learn something new. I should not be hurt by the study.
Compensation/Costs: It will not cost anything to participate, and I will not receive anything to participate in the study.
Confidentiality: At no time will my name be used to report or discuss answers. Questionnaires and tapes will be destroyed after the completion of the study.
Voluntary Participation/Withdrawal: I will participate because I want to not because anyone says I should (it is voluntary). I can choose not to participate, and my teacher or advisor can't punish me or give me a lower grade. I understand that I can quit participating at any time.
